Birth Announcement

Deadline for submission for the Sunday edition is 1 p.m. the Wednesday before publication. A submission constitutes this form and one photo. Announcement requested more than 30 days after the birth may be rejected at editor’s discretion. The name and phone number of a contact person, must be included on this form. Photo Identification and signature of mother and father must be provided before announcement will be published.
